With this document, Medical FX LLC informs about the location, composition, owner, processor and purpose of collecting personal data processed using the Medical FX - Patient software product. Informs about third parties to whom such personal data is transferred, the rights of the subject of personal data and other information necessary for you to provide your unambiguous and informed consent to the processing of personal data.

When you register or activate an account created by a medical institution in the Medical FX - Patient mobile application, you must consent to the processing of personal data in order to gain access to services and functionality. We cannot provide functionality or services through the "Medical FX - Patient" mobile application, unless you have previously obtained your consent to the processing of personal data.

Collection of personal data

The software product "Medical FX - Patient" stores only the information that was entered into the system when interacting with the software product: self-entry of data through the user's account, when entering information by a doctor or other specialist providing medical care or medical services, etc. when contacting a call center.

Processing of personal data

Your personal data is processed by Medical FX LLC, which owns all the necessary and sufficient rights to administer and provide access to the mobile application “Medical FX - Patient” on Google Play and the App Store.

Medical FX LLC is the owner of the personal data processed in the software product "Medical FX - Patient".

Location of Medical FX LLC: Doctor's Lane 2, Tbilisi, Georgia.

Composition of personal data

Full name, gender, date of birth, passport data, state identification number of a person, place of registration and / or place of residence, photo, email address, telephone number, insurance policy data, privileged categories, related persons (name, gender, date of birth , a document certifying the connection), data entered into the software product “Medical FX - Patient” and information about actions and IP addresses performed in the system;

Composition of sensitive data

In order to provide a quality service, we may collect sensitive data relating to the state of health: appointments with a doctor and their status; reception-related information; medical information about the state of health, medical history, research, referrals, therapeutic measures.

Please note that the information is not collected by us independently, but is entered by you, your doctor or other person providing medical care or medical services.

If you find false information about yourself in the system, contact the health administration (a person who has a license to carry out business activities in medical practice), whose employee entered such information. You can also apply for the correction of false information in the "Medical FX - Patient".

We do not have the right or the technical ability to delete information entered in state registers, in order to change such information you need to contact the relevant state authorities.

If you do not find the information that should be entered in the “Medical FX - Patient”, then you need to contact your doctor or healthcare institution (a person who has a license to carry out business activities in medical practice), whose employee did not enter the following information.

Gaining access to functionality

When using “Medical FX - Patient” on a device, certain functions require access to some data on the device. List of possible permission requests on your device:

● bluetooth (connection of additional diagnostic devices, headset);

● camera and microphone (for barcode scanner, tag scanner, photographing necessary documents, making a video call with a doctor);

● file storage (for uploading files or photos);

● geolocation (required to find the nearest institution);

● calendar (addition of scheduled events).

You can prevent “Medical FX - Patient” from accessing such data in the device settings. If you do not provide permission in such cases, we will not be able to provide the relevant functionality.

Purpose of collecting personal data

Your personal data is collected in order to provide quality and timely services, to ensure the full functioning of "Medical FX - Patient". This applies not only to the system as a whole, but also to individual services that you can choose from:

● an appointment with a doctor;

● maintaining an electronic medical record;

● storage of information;

● providing recommendations or other useful information, etc.

Informing

From time to time we will send you informational messages from us, i.e. calls, SMS, e-mail, etc. This is necessary for registration, authorization and confirmation of certain actions in “Medical FX - Patient”. You cannot opt out of such actions, as it is necessary for the normal functioning of the application.

We may also send you information that you may need or are interested in in the field of medicine and pharmacology/pharmacy. You can opt out of receiving other informational communications by contacting our team.

Saving photos

You decide whether to upload a photo or not. If you choose to do so, your photo will be stored in the Medical FX - Patient account system. We guarantee that this is the only way to use your image.

Place of storage of personal data

Personal data is stored in the database "Medical FX - Patient".

Medical FX LLC, with the consent and instruction of the user, is a young man and a person who determines the purpose of processing personal data, establishes the composition of this data and the procedures for their processing.

Protection of personal information

We take all possible organizational and technical measures to prevent information leakage - illegal or unauthorized access of any third parties to it.

"Medical FX - Patient" has all the necessary measures to protect information.

We use depersonalization and encryption of personal data during transmission and storage. We provide functionality and organizational measures that do not allow direct or indirect identification of a person. We anonymize the data in full, which does not lead to the inability to provide services to users.

Our employees get access to your data (any) only in exceptional cases, for example, if you contacted our call center yourself, based on a court decision, etc. Such employees must sign a written obligation not to disclose personal data that they were entrusted with or became aware of in connection with the performance of their professional or official duties. Each fact of access to personal data is recorded and stored.

Transfer of personal data

All personal data (both general and sensitive) entered into “Medical FX - Patient” can only be transferred to healthcare institutions and doctors licensed to practice medical practice (i.e. licensed to do so) to obtain by you medical care or health care services. The data may be provided to the authorized body in accordance with applicable law, as well as to the owners of other registries created in accordance with the law, if information about you must be transferred there and corresponds to its purpose (for example, state registries);

The data may be transferred to other persons, identified by you personally, when contacting such persons. That is, with the help of "Medical FX - Patient" making an appointment, registering the receipt of certain services, concluding an insurance contract, performing other actions, etc. At the same time, please note that the transfer is possible only in connection with your active actions (i.e. your request always precedes the transfer of data);

Medical institutions interacting with “Medical FX - Patient” may transfer data to recipients of mandatory reporting of medical institutions, which generate appropriate reports in the prescribed form, where there may be personal data. The use of such reports is clearly regulated, their transmission takes place in any case, regardless of the use or refusal to use information systems (including "Medical FX - Patient").

We also draw your attention to the fact that in order to carry out mailings (both authorization and informational), your phone number can be transferred to persons with whom Medical FX LLC has signed relevant agreements. These persons do not receive any additional information about you, except for your phone number. They also provide an undertaking not to carry out any mailings that are not agreed with Medical FX LLC and do not correspond to the purposes indicated in this message. Your phone number will not be matched against other databases, regardless of their owner.

In the cases described above, users are not additionally informed about the transfer of data.

Related persons

It is often convenient to delegate an appointment, its difference, etc. to relatives or other trusted persons. However, these actions require confirmation. It is up to you to decide whether you want to associate with a person who allows you to view certain categories of information about you and / or access information about such person. If you and your person have a corresponding wish, the doctor during the appointment can verify the relationship you have established (by checking the documents, and in the absence of documented grounds, by obtaining proper confirmation).

By connecting a related person, you may create, view, cancel and otherwise manage the appointment of a relative or person of whom you are the guardian, subject to the consent of both persons or the corresponding rights as a guardian;

In the case of representation (legal representation by parents of children, guardianship or guardianship), by providing the doctor for verification with official documents certifying the fact of such representation and, accordingly, confirming the availability of authority, you can view all information (including medical) and perform all necessary actions in account of your minor child, view all information (including medical) and perform all necessary actions in the account of the ward.

User rights

As a subject of personal data, you are endowed with a set of rights:

● know about the sources of collection, the location of their personal data, the purpose of their processing;

● receive information about the conditions for granting access to personal data, in particular, information about third parties to whom your personal data is transferred;

● access your personal data;

● receive an answer about whether your personal data is being processed, as well as receive the content of such personal data;

● present a reasoned demand to the owner of personal data with an objection to the processing of their personal data;

● submit a reasoned request to change or delete their personal data by any owner and manager of personal data, if this data is processed illegally or is unreliable;

● to protect your personal data from illegal processing and accidental loss, destruction, damage due to deliberate concealment, failure to provide or untimely provision of them, as well as to protect against the provision of information that is unreliable or discredits the honor, dignity and business reputation of an individual;

● withdraw consent to the processing of personal data;

● know the mechanism of automatic processing of personal data;

● in defense of an automated decision that has legal consequences for him.

Term for processing and deleting personal data

Personal data is stored in “Medical FX - Patient” for the entire time that you use the system and 5 years after the last access to such data by you or your doctor. In the event that personal data is part of an electronic medical record created in accordance with applicable law, the storage period for such an electronic medical record is 5 years, unless another storage period is determined by law.

If we receive information, supported by relevant evidence, about entering false information into the system, violation of the User Agreement or this privacy policy when using “Medical FX - Patient”, use of third-party means of communication for registration or registration of third parties without sufficient authority, Medical FX LLC reserves the right to suspend an account for which such information has been found, without prior notice, for the duration of the audit.

According to the results of the audit, in case of establishing the fact of entering false information into the system, violation of the User Agreement or this privacy policy when using “Medical FX - Patient”, use of third-party means of communication for registration or registration of third parties without sufficient authority (or the inability to refute such facts), a decision may be made to delete inaccurate information or to delete the account regarding which such facts were discovered, in general, without the possibility of further recovery (in case of entering inaccurate data about the means of communication (used as one of the identifiers in “Medical FX - Patient”),

full name, or if the amount of inaccurate information left after deletion does not allow the formation of a user account), which the person who created such an account can be reported if we have information about the means of communication with it.

Blocking or deleting an account in “Medical FX - Patient” does not affect the information entered in the public e-health data registries.

Final provisions

Changes and additions to the privacy policy are made by issuing a new edition in electronic form, published in “Medical FX - Patient”.

The date of entry into force of the new edition of the privacy policy is the date of their publication on the website "Medical FX - Patient". We will definitely notify you of such changes by requesting a new consent to the processing of personal data (in accordance with the terms of the new edition of the privacy policy).

If such a new edition is not accepted, we ask you to stop using “Medical FX - Patient” and remind you that you have the right to demand the destruction of your personal data entered in “Medical FX - Patient” in accordance with the procedure established by law.
