USER AGREEMENT
The User Agreement (hereinafter referred to as the “Agreement”) is set forth below and is addressed to an indefinite circle of persons, users of the “Medical FX - Patient” mobile application and determines the procedure for use.
The person with all necessary and sufficient rights to administer and provide access to “Medical FX - Patient” is Medical FX LLC.
This Agreement is a public contract in accordance with applicable law.
The use of “Medical FX - Patient” is considered an unconditional acceptance (acceptance) of this Agreement.
Duration of the Agreement
“Medical FX - Patient” — software products, as a result of computer programming, designed to organize the process of providing medical care and/or medical services, access to which is carried out through the mobile application “Medical FX - Patient”, which is available for download on Google Play and App Store and is provided for the personal use of users.
User — an individual with full civil capacity, and using “Medical FX - Patient”, regardless of the presence or absence of an account.
Account — a set of information about the user (the person whose legal representative he is) and authorization data (phone number, login, password) entered by the user in “Medical FX - Patient” during registration or activation and when using the system.
General terms of use “Medical FX - Patient”
- Before using “Medical FX - Patient”, the user must read the Agreement and accept its terms.
- The user can register with “Medical FX - Patient” or register the person of whom he is the legal representative (children, persons for whom guardianship has been established, etc.).
- Registration or activation in “Medical FX - Patient” is possible only if the user consents to the processing of his personal data (or the personal data of the person whose legal representative he is).
- The terms of this Agreement apply to all users of “Medical FX - Patient” — both registered and non-registered.
- If the user does not agree with the terms of this Agreement, he must stop using “Medical FX - Patient”.
Features “Medical FX - Patient”
- “Medical FX - Patient” is designed to organize the process of providing medical care and/or medical services. We are not directly involved in providing medical care or services, do not influence the work of healthcare institutions, laboratories, pharmacies, or specialists, and have no control over their activities. “Medical FX - Patient” only provides a platform that can be used for communication between healthcare institutions and you as a patient.
- You choose which Medical FX - Patient services to use (e.g., appointment booking, electronic records, insurance, etc.).
- “Medical FX - Patient” stores only the information entered by you personally or by specialists providing you with services.
- All information posted in “Medical FX - Patient” is for reference only. The system cooperates with licensed medical organizations and provides a communication platform but does not interfere in your relationships with them.
- Information about the user is stored only upon account creation and may form part of an electronic medical record. Use and transfer of such data are regulated by the “Privacy Policy”.
Rights and obligations of the user of “Medical FX - Patient”
- The user guarantees to:
- Enter only truthful and up-to-date data.
- Confirm the accuracy of medical information and understand the risks of entering false data.
- Keep information up to date.
- Notify the “Medical FX - Patient” team immediately about loss or change of phone number.
- Keep passwords confidential and use secure devices.
- Acknowledge full responsibility for all actions performed under his authentication means.
- Accept that any actions with his credentials are considered his own unless proven otherwise.
- In case of discontinuing use, submit a request for deletion of personal data via email support.mfxp@accuracy-med.com or through the website feedback form www.accuracy-system.com.
- If the user believes that “Medical FX - Patient” contains information violating his rights, he must report this with supporting evidence.
- If false information or violations are detected, the organization may block the account for verification.
- Upon confirmation of violations, the organization may delete false information or the account without the possibility of recovery.
- To ensure quality, usage limits may be imposed (e.g., appointment frequency).
- The organization is not responsible for information entered by users or third parties in “Medical FX - Patient” and does not compensate for any direct or indirect damages.
Statistical information
- We may store statistical information when using “Medical FX - Patient” to improve user experience and analyze service usage.
- All collected information is generalized, not personalized, and 100% anonymous.
- Any statistical information that we collect is completely anonymous and excludes the possibility of identifying individuals whose activity information could be included in such statistics.
- We may share anonymous information about the use of “Medical FX - Patient”, current user needs (necessary services, services, etc.), number of visits, etc. with our partners to better understand the needs of the healthcare industry and the use of information systems in medicine.
Other conditions
- “Medical FX - Patient” has the right to make changes to this Agreement. All changes made to the Agreement will be available in the form of a new version of the Agreement posted on the website or in the mobile application.
- In the event that significant changes are made to the Agreement, we will additionally notify of these changes by e-mail or by a notification in the user's account. All changes to the Agreement come into force from the moment of publication. By using “Medical FX - Patient”, the user confirms acceptance of the new terms of the Agreement as amended, valid at the time of using “Medical FX - Patient”.
- The inclusion of a telephone number or other means of communication in “Medical FX - Patient” means that the user of Medical FX LLC has the right to send him informational messages, i.e. calls, SMS, e-mail, etc. This is necessary for registration, authorization, confirmation of certain actions in “Medical FX - Patient” or obtaining information that may be required or interesting for a user in the field of medicine and pharmacology / pharmacy.
